JOB SUMMARY: The Women’s Health Case Manager is prayerfully responsible for coordinating wellness and case management needs in our residential program. The responsibilities can be broken down into three areas: Women’s Health, Resident Case Management, and Organizational Administrative Duties.
Women’s Health
- Teaches and equips residents to move into various life skills in order to thrive in independent living.
- Record and file all appointment records and medicine administration records in individual resident med folders
- Update and fill inventory for all necessary medications as prescribed or doctor-recommended over the counters; assuring accurate administration of those meds.
- Schedules and transports clients to ongoing medical and therapeutic appointments as needed.
Resident Case Management
- Facilitate or assist with resident office tasks as coincides with the program map.
- Coordinates activities, communication & appointments with outside referral sources for client care including legal, financial, educational, housing and behavioral health resources
- Assists residents in obtaining vital records, health insurance, SNAP benefits, and other case management tasks as needed.
Organizational Administrative Duties
- Maintain medical appointment calendar, implementing direction from Program Director.
- Represent Eden’s Glory at public events. Participates in educational/awareness activities for the community related to Eden’s Glory.
- Monitor expenses within budgetary limits.
- Attends weekly worship, staff meetings, family Friday lunches, and quarterly retreats.
- Schedule and participate in a monthly four-hour prayer retreat.
- Other duties as may be requested by Executive Leadership

- Maintains own licensure & certification, if applicable.

- Assures a therapeutic environment for clients, including client rights, dignity, respect, privacy, education, and participation in program initiatives.
- Utilizes a client-centered, strengths-based, and problem-solving approach.
- Creates an environment conducive to client growth utilizing a client-centered, strengths-based, and problem-solving approach through a trauma-informed lens.

- Demonstrates ability to perform well under stress.
- Communicates complaints and/or suggestions professionally and through appropriate channels.
- Fulfills responsibilities in a dependable manner and uses time effectively.
- Recognizes personal as well as staff educational needs and shows motivation to meet these deficits.
- Demonstrates highly effective communication skills.
- Pursues opportunities and annually attends a minimum of 15 Continuing Education Units, assuring topics related to the work and ministry at Eden’s Glory.
- Adheres to safety guidelines and procedures at all times.
- Practices self-care and pursues counseling or mentorship on a quarterly basis, minimum.
- Prioritizes and participates in a four-hour prayer retreat each month.
Skills:
Demonstrates organizational, verbal, interpersonal, customer relations, mathematical, analytical, grammar/spelling, reading/comprehension of written instructions, ability to follow verbal instructions, and computer skills.
Job Knowledge:
Knowledge of local/global human trafficking issues including common processes, prevalence, legal processes, networks of stakeholders, and appropriate interventions.
Education/Experience:
- Graduation from a state approved school of social work or other appropriate related field.
- Master’s degree preferred, Bachelors required.
- Previous exposure/work related to field of human trafficking, social service, clinical therapy, or other related human services.
- Previous program management experience preferred.
